This mostly occurs because the increase in CO2 at the level of renal tubular cells stimulates the secretion of H+ ions and the elimination of more acid urine, while HCO3 ions are reabsorbed.
In the case of renal impairment, acidbase balance is not efficiently restored, resulting in prolonged blood acidosis and suffering for the whole body.
When renal failure occurs in COPD patients, the compensatory role of kidney in respiratory acidosis may be less effective, resulting in a reduced ammoniagenesis and titratable acidity production with consequent limited rise of serum bicarbonate and a more severe acidosis. Clinical reports have demonstrated that bicarbonate levels in these patients are inversely related to survival and that concomitant renal failure is predictive of death and risk of exacerbation .

Study Design And Selection Of Participants
The analyses were based on all hospitalized patients enrolled in the Genetic and Inflammatory Markers of Sepsis study. GenIMS is a large, multicenter, observational cohort study of subjects with CAP presenting to the emergency departments of 28 teaching and nonteaching hospitals in the United States between November 2001 and November 2003. Eligible patients were ⩾18 years old and had a clinical and radiological diagnosis of CAP using criteria by Fine et al. Definition Of Acute Kidney Injury
We classified AKI using the maximum RIFLE stages, as proposed by the Acute Dialysis Quality Initiative at any time during hospitalization. The RIFLE stage was determined based on the worse of either serial serum creatinine or urine output. For patients with no known premorbid creatinine and no known medical history of chronic kidney disease, we estimated premorbid creatinine using the Modification of Diet in Renal Disease equation, as recommended by the Acute Dialysis Quality Initiative. We then selected the lower creatinine value from either the hospital admission creatinine or the Modification of Diet in Renal Disease creatinine as the baseline value. Patients were classified as stage Risk, if serum creatinine was 1.5 times the baseline creatinine, or urine output < 0.5 ml/kg/h for 6h stage Injury, if serum creatinine was twice the baseline, or urine output < 0.5 ml/kg/h for 12 h stage Failure, if serum creatinine was thrice the baseline, or creatinine ⩾4 mg/dl with an acute rise > 0.5 mg/dl, or urine output < 0.3 ml/kg/h for 24 h, or anuria for 12 h.

Reports Of Spider Bites From India
Spider bites are inadequately reported in India, which is one reason for lack of experience in dealing symptomatic bites.
In 2014, the Journal of Postgraduate Medicine reported a case of spider bite by Poecilotheria species from south India. A 32-year-old man was admitted to the emergency department of the Christian Medical College, Vellore, Tamil Nadu, India three hours after a spider bite to his right hand. The patient complained of headache and mild blurring of vision along with pain and swelling in right hand. He was treated with analgesics ), limb elevation, and tetanus toxoid and discharged after 12 hours as the swelling and pain subsided. After seven days, he was relieved of all the symptoms.
The spider fell on him while was sitting under a tree and bit him. He also brought the killed spider with him, which was later identified as Poecilotheria species belonging to the group of big hairy spiders commonly called as a “tarantula”. Usually, such spiders live in the cracks and holes of tall forest trees. Intense pain, swelling at the bite site and muscle cramps are the common symptoms of bites by Poecilotheria species.
However, there are also reports of serious complications following a spider bite from India. For example, a case of a brown spider bite causing intravascular hemolysis and acute renal failure or acute kidney injury was reported from eastern India in 2014.
