In some areas, blood tests have shown that 20 to 40 percent of farm workers have antibodies that react with antigens responsible for Farmer’s Lung. This test indicates that people have had at least an initial exposure to moldy dust.
The disease is most common in regions with wet weather at harvest time. How Can Aspergillosis Be Prevented
Due to the prevalence of aspergillus mold in the environment, it is very difficult to avoid exposure. It is best to avoid locations with excessive amounts of dust or mold, such as construction sites or compost piles. People with weakened immune systems or mold allergies should avoid activities such as gardening or lawn mowing. If exposure to airborne dust or mold is likely, considering wearing a face mask or N95 mask. In some cases, your doctor might recommend the use of an antifungal medicine to prevent infection.

Prevention And Patient Resources
After major flooding or other exposure to water damage and mold, talk with your immunocompromised patients about how to protect themselves from mold:
- Mold grows where there is moisture, usually within 24-48 hours of flooding. Even if it is not visible, it is often present.
- Immunocompromised people should not enter moldy buildings or help clean up mold.

Top Tips For Controlling Mold
It’s impossible to get rid of all mold and mold spores in your home, but because mold spores can’t grow without moisture, reducing moisture in your home is the best way to prevent or eliminate mold growth. If there is already mold growing in your home, it’s important to clean up the mold and fix the problem causing dampness. If you clean up the mold but don’t fix the problem, the mold will most likely return.
Following is some advice for reducing moisture throughout the home with specific tips for the areas most prone to dampness and mold growth:
Around the house:
In the kitchen:
- Use exhaust fans to move moisture outside whenever you are cooking, washing dishes, or cleaning.
- Turn off certain appliances if you notice moisture on windows and other surfaces.
- Check for leaks around the kitchen sink, refrigerator ice makers, and other sources of water. Repair if necessary.
- Empty and clean refrigerator drip pans if necessary.
In basements and crawl spaces:

- Vent your clothes dryer to the outside.
- Make sure the vent is clear of obstructions, such as lint, and that there are no holes that leak air. If the vent duct is damaged, replace it with a metal duct. Have the duct cleaned at least once a year.
- Avoid leaving damp clothes in the laundry basket or dryer. Wash and dry them promptly.
Sputum Examination And Potassium Hydroxide Stain
This study may show fungal hyphae or yeasts. However, the results must correlate with the clinical situation, because saprophytic colonization occurs in the oropharyngeal or respiratory tract of some patients and may not necessarily indicate invasive infection.
Carefully transport, process, and culture specimens that may be contaminated by bacteria, may be saprophytic yeasts endogenous to the oral cavity, and may be airborne conidia of saprophytic fungi.
The diagnosis of pulmonary cryptococcal infection is confirmed if the organism is grown in culture from sputum or BAL fluid in a patient who has clinical symptoms and radiographic finding compatible with cryptococcosis.
Histoplasmosis is definitively diagnosed by growth of the organism in sputum BAL fluid, lung tissue, or mediastinal nodes can be cultured.
In pulmonary sporotrichosis, the recovery of the fungi by culture of sputum and/or positive bronchoscopy are required for diagnosis.
Cultures from sputum samples collected by fiber optic bronchoscopy are not valuable for the diagnosis of pneumonia by Candida. To make the diagnosis, a biopsy is required to demonstrate tissue invasion. Colonization of the respiratory tract by Candida is very frequent in critically ill patients with mechanical ventilation, but pneumonia by Candida is extremely rare because the innate defense mechanisms of the lungs make them relatively resistant to candida invasion.

How Does Mold In Lungs Cause Illness
Mold can cause respiratory illness in a number of ways.
First, it causes irritation to the delicate lining of the bronchi and the tiny bronchioles in the lungs. Inflammation and irritation of the bronchioles make it difficult to get enough air through, causing wheezing and labored breathing. Asthma attacks may occur. Infection of the bronchi, called bronchitis, can also result from mold in the air passages.
Second, microscopic particles of mold that are not expelled by coughing can lead to pneumonia when they become lodged in the lungs. Symptoms of pneumonia range from moderate to severe and include fever, shortness of breathing, coughing, and chest pain when you cough or breathe.

What Causes Aspergillosis
In most cases, aspergillosis is caused by a type of mold called Aspergillus fumigatus. Aspergillus mold can often be found on dead leaves, compost piles and other decaying vegetable matter, stored grain, and even foods and spices. The mold spores may be carried indoors on shoes and clothing and can grow on carpeting. Window unit air conditioners are susceptible to mold growth if filters are not kept clean, and water does not drain properly from the unit. Mold Around The House
While antihistamines can address many of the symptoms of a mold allergy, they are often not a sufficient standalone response to mold. Antihistamines can have sedating effects on users and adversely interact with other prescription and over-the-counter medications, so it is best not to rely on them entirely. Instead, after identifying signs and symptoms of a mold allergy, work to reduce the presence of mold in your home. This process often involves optimizing air filtration systems, cleaning bathrooms regularly, and making basement living spaces as hypoallergenic as possible by limiting porous surfaces, such as carpeting, where moisture and attendant mold can build up. While it is impossible to eliminate mold exposure, especially outdoors, you can significantly mitigate mold exposure within the home.

What Is Farmer’s Lung
Farmer’s Lung is an allergic disease usually caused by breathing in the dust from moldy hay. However, dust from any moldy crop – straw, corn, silage, grain, or even tobacco – can also cause Farmer’s Lung.
The technical name for Farmer’s Lung is “extrinsic allergic alveolitis“, “hypersensitivity alveolitis” or more generally “hypersensitivity pneumonitis”. The “-itis” word ending means inflammation, so “alveolitis” means inflammation of the alveoli. “Pneumonitis” means inflammation of the lungs . As the name suggests, Farmer’s Lung is caused by inhaling certain allergy-causing dust found in some agricultural operations. A recent study shows that exposure to organochlorines and carbamate pesticides may also be risk factors for farmer’s lung. However, “extrinsic allergic alveolitis” or “hypersensitivity pneumonitis” are general medical terms that include other lung conditions resulting from exposure to allergy-causing dusts found in different kinds of workplaces or locations. Nonculture Methods For Detecting Fungal Infections
These provide a more rapid and sensitive test when compared with culture methodology. Various antigen detection assays, such as galactomannan enzyme immunoassay for detection of invasive aspergillosis, are now in clinical use. Polymerase chain reaction âbased assays are also available for detecting various pathogens, including Aspergillus, Histoplasma, and Candida species.
Comparison of these assays show equal specificities for all 3 assays in the detection of Candida species. PCR-based assays are most sensitive compared with ELISA and latex agglutination .
The newest available testing in the diagnosis of blastomycosis is an enzyme immunoassay performed on urine or serum that detects a cell wall galactomannan antigen found in B dermatitidis. However, the cross reactivity with H capsulatum is close to 100%. Therefore, in patients with blastomycosis, the sensitivity or specificity of these tests is not clear.
For Aspergillus species antigen, galactomannan assay findings may be positive in the blood very early prior to clinical suspicion of invasive fungal infection and may be of use in monitoring and preemptive treatment in high-risk populations.
The molds causing mucormycosis, which belong to the subphylum Mucoromycotina, do not contain galactomannan or beta-glucan in their cell wall. Therefore, mucormycosis cannot be diagnosed with the galactomannan or beta-glucan assays.
